Semaglutide vs tirzepatide: what actually differs?

By Toven Health · · Updated · 4 min read

THE SHORT ANSWER

Semaglutide and tirzepatide are different medicines. Tirzepatide produced greater average weight loss in one major head-to-head trial, but that result does not tell you which treatment fits your health history or establish the performance of a compounded formulation.

How are semaglutide and tirzepatide different?

Semaglutide activates the GLP-1 receptor. Tirzepatide activates both GIP and GLP-1 receptors. These names describe biological signaling pathways; the number of pathways alone is not a personal treatment recommendation. The prescribing information explains how each medicine is used and which patients should avoid it.

What did the weight-loss comparison show?

SURMOUNT-5 compared weekly injections in 751 adults with obesity who did not have diabetes. At 72 weeks, average weight change was about -20.2% with tirzepatide and -13.7% with semaglutide. This was an open-label, randomized trial funded by Eli Lilly, the manufacturer of tirzepatide. Participants received the trial’s specified treatments and dose escalation, not an assortment of wellness-clinic formulations.

An average is not a forecast for an individual. The study population, length of follow-up, treatment schedule, and reasons people stopped treatment all matter when interpreting results. This comparison does not establish that a B12 blend or a marketed microdose program will deliver the same outcome.

Start with the exact product, not just the molecule

Wegovy contains semaglutide; Zepbound contains tirzepatide. Their labels include weight-management uses for eligible patients, alongside diet and physical activity. An ingredient name on a clinic menu does not identify the manufacturer, formulation, delivery device, or approved indication. Ask which exact product a clinician is discussing.

  • Is this an FDA-approved product or a compounded prescription?
  • Which indication and eligibility criteria apply to me?
  • Who is prescribing, dispensing, and providing follow-up?
  • What is included in the quoted price, and what could change over time?

Side effects and medical history belong in the comparison

The Wegovy and Zepbound labels describe gastrointestinal side effects and potentially serious risks. Both carry boxed warnings about thyroid C-cell tumors observed in rodents, with unknown relevance in humans, and contraindications for a personal or family history of medullary thyroid carcinoma or MEN 2. These are examples, not a complete safety review.

Bring your medication list, previous treatment experience, pregnancy plans, and relevant diagnoses to a licensed clinician. Ask for the current medication guide for the exact prescribed product. A short comparison article cannot establish eligibility or provide instructions for starting, combining, or switching medicines.

Where compounded semaglutide and tirzepatide fit

FDA-approved drugs and compounded preparations are not interchangeable regulatory categories. FDA does not approve compounded drugs or review them for safety, effectiveness, and quality before marketing. Compounding may serve a patient whose medical needs cannot be met by an approved drug, subject to applicable requirements.

For a useful comparison, keep two questions separate: which medicine is appropriate, and which exact formulation is being supplied? A study of a manufactured product cannot by itself validate a different preparation. Our formulation guide explains B12 additives and microdose labels in more detail.

A better way to prepare for a consultation

Write down the outcome you want to discuss and the practical barriers you anticipate. For example, you might ask about maintaining treatment through travel, getting timely advice about side effects, or budgeting for follow-up. This turns a broad “which is better?” question into a discussion about a workable care plan.

Ask how progress would be assessed and what would prompt a change in approach. Request a written explanation of ongoing costs and the contact route for questions. Common questions

Does greater average weight loss mean tirzepatide is best for everyone?

No. Trial averages do not determine individual eligibility, tolerability, access, or the right care plan.

Does the comparison apply to B12 blends or microdosing?

The cited trial studied its specified regimens. It does not validate other formulations or schedules.
